What is GERD?

Gastroesophageal Reflux Disease (GERD) is a common disorder in which stomach acid moves upward in the esophagus and causes heartburn.

This acid reflux can damage the esophagus lining. If it causes frequent symptoms and injuries, reflux develops into GERD.

What is GERD?

GERD, or gastro-oesophageal reflux disease, is a condition where stomach acid flows back into the oesophagus repeatedly, causing irritation. Unlike occasional heartburn, GERD is chronic. Left unaddressed, it damages the oesophageal lining and raises the risk of complications including Barrett’s oesophagus and oesophageal cancer.

The most common symptoms are heartburn, acid regurgitation, and a sour taste in the mouth. Many people also experience a chronic dry cough, hoarseness, difficulty swallowing, bloating, and disrupted sleep. Some have no heartburn at all but present with throat or respiratory symptoms, which makes GERD easy to miss or misattribute.

The primary cause is a weakened or dysfunctional lower oesophageal sphincter, the valve between the oesophagus and stomach. Contributing factors include obesity, a diet high in processed and fatty foods, frequent overeating, chronic stress, smoking, alcohol, certain medications, and a hiatal hernia. Gut dysbiosis and low stomach acid, often overlooked, also play a significant role.

Not always. A substantial number of GERD cases involve low stomach acid rather than excess acid. The symptoms feel identical, but the mechanism is different. Treating low-acid GERD with acid-suppressing medication makes the underlying problem worse over time. This is one reason a root-cause assessment matters before defaulting to long-term antacid use.

Proton pump inhibitors and antacids suppress symptoms but do not address the reason reflux is occurring. Long-term PPI use is associated with magnesium and B12 deficiency, increased susceptibility to gut infections, and accelerated gut dysbiosis. Most gastroenterology guidelines recommend using them for the shortest necessary period alongside lifestyle intervention, not indefinitely.

A clinical diagnosis is made based on symptom history. Where symptoms are atypical or severe, an endoscopy, oesophageal pH monitoring, or manometry may be used to assess the extent of damage and rule out other conditions. Functional medicine assessment also looks at gut microbiome health, stomach acid levels, and dietary patterns that standard workups typically skip.


In many cases, yes. Dietary changes, weight management, stress reduction, and correcting underlying gut dysfunction significantly reduce or eliminate symptoms for a large proportion of patients. How much reversal is possible depends on how long the condition has been present and whether structural changes like Barrett’s oesophagus have developed.
