What is Crohn's disease?

Crohn’s disease is a chronic inflammatory condition that can affect any part of the digestive tract, from the mouth to the anus, though the small intestine and the junction between the small and large intestine are most commonly involved. Unlike ulcerative colitis, which is confined to the colon, Crohn’s inflammation penetrates through the full thickness of the gut wall, which is why complications like fistulas, abscesses, and strictures are more common.

Persistent diarrhoea, cramping abdominal pain, unintended weight loss, fatigue, and rectal bleeding are the most frequent symptoms. Many patients also experience joint pain, mouth ulcers, skin lesions, and eye inflammation, reflecting the systemic immune involvement. Symptoms cycle between active flares and periods of remission, which often leads to years passing before a correct diagnosis is made.

Intestinal tuberculosis and Crohn’s disease present almost identically on colonoscopy, imaging, and biopsy. In a country where TB remains endemic, Crohn’s is routinely assumed to be TB first. Studies show that 43% of Crohn’s patients in India received anti-tuberculosis therapy before getting the correct diagnosis. This delay worsens long-term outcomes and increases the risk of surgical complications significantly

The precise cause remains under investigation, but the current understanding points to a combination of genetic susceptibility, an overactive immune response, gut microbiome dysbiosis, and environmental triggers.
